Prakashini, V.R.K. Rao Kasturba Medical College, Manipal, India Background: One of the commonest extrapulmonary site for tuberculosis is genitourinary system. Definite diagnosis of tuberculosis is by urine acid fast staining or biopsy specimen, which is time taking and thus the final diagnosis may be delayed. Though the IVU features are nonspecific, presences of multiple patterns suggest tubercular culture or biopsies for confirmation there by possible early initiation of treatment. Here is a retrospective study to analyze the spectrum of imaging findings of tuberculosis affecting the kidney, ureter, bladder on excretory urography. Methods: The study was conducted retrospectively in a teaching hospital between 2002 and 2006. Proven cases of urinary tuberculosis with excretory urography were collected. Total 50 IVU examinations were reviewed for various imaging findings and three specific imaging patterns. Multiple findings were considered to be present when single finding seen involving multiple sites or co-existing multiple findings. Results: Most common finding was hydrocalycosis, hydronephrosis or hydroureter secondary to strictureseen in 36 cases out of 50, followed by parenchymal cavities and irregular or moth eaten calyces 6 each, calcifications and scarring in 5 cases each, autonephrectomy in 4 cases and thimble bladder in 12 IVU imaging. Multiple findings with three imaging patterns are noted in 36 72% ; cases. Various imaging pattern includes multiple stricture sites seen in 25 out of 50, a single stricture with one other imaging finding in 7 cases or autonephrectomy with one another findings other than stricture seen in 4 cases. Conclusion: Spectrum of imaging findings of urinary tuberculosis was seen on IVU in our study. Although each finding is nonspecific, suspicion of tubercular etiology should be made when there are multiple findings with three imaging pattern on excretory urography and tubercular culture or biopsies should be suggested.

The trials. It can thus be inferred that these tables reflect the exact numbers in the study. The probabilities used have been applied exactly, with no attempt at smoothing or checking for consistency between anakinra and placebo. For example, consider the transitions for the improvement from state 3 to state 1. The trial results are consistent 2 0.024, p 0.877 ; with a null hypothesis that the proportions are the same, and so it is entirely plausible that the `correct' transition probability should be higher for anakinra than for placebo. However, for the question of overall improvement from state 3, the model uses probabilities of 0.605 for anakinra against 0.381 for placebo. Where these zeros occur also in trial 0564, the given transition becomes impossible at.
There are also doubts if the so-called re-treatment regimen for patients with previous anti-TB treatment for at least one month ; is needed Connolly et al., 1999; Wilkinson et al., 1998 ; . One may also argue that for re-treatment patients it would be more cost-effective to get drug resistance results though this is a lengthy procedure and not possible in many rural areas ; than to automatically give them the re-treatment regimen. In other words, a stronger case should be made for making drug susceptibility testing available to a much larger number of TB patients.

Occasionally, granulomas coalesce to form mass like lesions, particularly in the region of the chiasma, floor of the third ventricle, and pituitary stalk. Clinical symptoms correlate with the location of the lesion on MR images. Furthermore, it was found that these lesions recur frequently Ando et al., 1985 ; . Disease entities that can involve the basal leptomeninges and mimic leptomeningeal sarcoidosis on imaging include granulomatous disease such as TB ; , Wegener granulomatosis and fungal meningitis, pyogenic meningitis, leptomeningeal lymphoma, demyelination, meningoangiomatosis, acute lymphocytic leukemia, and leptomeningeal carcinomatosis Brooks et al., 1982 ; . Dural epidural mass lesions have an imaging appearance similar to that of meningioma epidural lymphoma and are not associated with intraparenchymal extension. Dural epidural sarcoidosis probably represents blood borne deposits of the disease in the epidural spaces. The annual incidence of these lesions exceeds the annual incidence of meningiomas 2.3 per 100, 000 ; by 130 times. These lesions tend to be isointense with gray matter on T1-weighted MR images and hypointense on T2weighted images, and they enhance uniformly Figure 19 ; . This hypointensity on T2-weighted images has been reported previously Chaflenor et al., 1984 ; and is thought to be related to fibrocollagenous buildup. Unfortunately, this is not a unique finding. Eighteen percent of meningiomas typically fibroblastic or transitional types ; demonstrate low signal on T2-weighted images. The aim of this study was to identify prognostic factors for survival without recurrence after liver transplantation. Four-hundred and twelve patients transplanted for hepatocellular carcinoma were studied. Survival at 5 years was 55 %. In multivariate analysis, 6 variables were associated with survival without recurrence: diameter of the largest nodule, tumor differentiation, transplantation period, use of OKT3 or SAL, pre-operative portal or hepatic vein thrombosis, and number of tumors. Conclusion: This study confirms the prognostic value of tumor differentiation and suggests that use of OKT3 and SAL could be associated with survival without recurrence after liver transplantation for hepatocellular carcinoma and buy lasix. Primary care providers said they changed their patterns of specialty referrals to be more consistent with conservative treatment of acute back pain patients. Providers responding to our survey at the second site visits were asked: "Has the low back pain guideline led you to increase or decrease your referrals to the following services?" Overall, they reported that referrals to back classes, physical therapy, chiropractors where available ; , and dieticians increased, but that referrals to X ray, CT scan, MRI, neurosurgery, and orthopedics decreased or remained the same. In contrast with the primary care provider reports, neurosurgery and orthopedics specialists at the demonstration sites indicated that they continued to receive inappropriate referrals that represented an estimated 10 to 30 percent of the total patients referred to them. In interpreting these reports, the reader should keep two limitations in mind. First, the reported changes reflect the providers' perceptions of changes in their behavior, which may have differed from actual changes as measured with encounter data. Second, the providers who participated in the focus groups probably were not representative of all providers at the MTFs. These providers some of whom were members of the implementation teams ; were likely to be more knowledgeable about the guideline than others who did not participate in the focus groups, and they were also likely to be readier to adopt new practices stated in the guideline. One site changed its handling of chronic low back pain cases. The primary care clinics began to refer all persistent cases to the physical medicine clinic for assessment by a multidisciplinary team and referral to the appropriate specialist s ; and, if necessary, to permanent profiling or the MEB. Treatment of difficult cases was coordinated in weekly meetings of a multidisciplinary team. According to the site, this practice eliminated a chronic backlog of neurosurgery referrals and helped to standardize the treatment of chronic low back pain patients.
